Your first visit to San Juan Foot & Ankle Center establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. What to Bring

New patients should arrive 30 minutes prior to your scheduled appointment time to complete the new patient forms.  Please bring the following items to the appointment:

  • Insurance card(s) and Driver's License
  • List of all prescription and over the counter medications that you are currently taking and any allergies you may have
  • Any required co-payment
  • Medical and surgical history information​


Insurance

We accept and participate in MOST insurance plans, including Medicare and Medicaid. Since there are so many different insurance plans, it is the patients responsibility to understand their own policy and know whether podiatry is a covered benefit.  Patients should also contact their insurance to determine if we are in-network, or out-of-network. Co-payments are due at the time of service. As a courtesy, we will bill your insurance, and you are responsible for any unpaid balance not covered by insurance.
