What to Bring

New patients should arrive 30 mins prior to your scheduled appointment time to complete the new patient forms. Please bring the following items to the appointment:

  • Insurance card(s) and Driver’s License
  • List of all prescription and over-the-counter medications that you are currently taking and any allergies you may have.
  • Any required co-payment
  • Medical and surgical history information

What to Expect

During the first visit, we will obtain important background information, like your medical history, and give you time to get to know your doctor. Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. Educate yourself on your symptoms by reviewing the content on our Web site, and review the “What to Bring” section above. We look forward to your first visit.


Insurance

We accept and participate in most insurance plans, including Medicare and Medicaid. Since there are so many different insurance plans, it is the patient’s responsibility to understand their own policy and to know whether podiatry is a covered benefit.  Copayments are due at the time of service. As a courtesy, we will bill your insurance, and you are responsible for any unpaid balance not covered by insurance.
